I am requesting a waiver to allow my e-rate form 471 to be reviewed by USAC because I unfortunately missed the the deadline duty to a misunderstanding regarding how to add a contract to the form. By the time I was able to reach someone to help me with the form, the filing window had closed. I have never missed a filing window before. My confusion was a result of my lack of familiarity with the process for filing for category 2 funding. In the past I have only filed for category 1 funding and the process has a number of significant differences.
